Company Information

• Tax Identification Number
• Gross and net Revenue for the most recent year (Please bring a copy of your most recent Tax Return)
• Please submit a copy of your Fictitious Business Name Statement, if any. If a Corporation or LLC, please provide us with a copy of the Articles of Incorporation and/or Articles of Organization and a Current Statement of Information (dated within the last 12 months). For an LLC, also please submit a copy of the Operating Agreement. If a Partnership, LP, or LLP, please supply a copy of the Partnership Agreement and LP-1 or LLP-1, if applicable. If a Joint Venture, please supply a copy of the Joint Venture Agreement.

Personal Information

• Social Security Number
• Total amount of outstanding personal loans and mortgages
• Your personal annual Household Income for the most recent year (Please bring a copy of your most recent Tax Return)
• Total amount of personal assets, including cash, securities, real estate & autos

Note: A separate completed application is required for any person owning 20% or more of the business. Please complete all areas of the application to insure a prompt loan decision.
